Are you in need of Medical Assistance? We would like to help.

Program eligibility/requirements
• Families must be unable to pay for a medical procedure
• Children or adults with extreme medical conditions
• Citizen of Guyana
• Beneficiary/guardian must be willing to sign a release form allowing Food For The Poor and its affiliates to utilize their stories, photos, videos and any other media for fundraising and reporting purposes.

Applicant must submit

1. Letter requesting assistance
2. Photograph of patient/applicant
3. Copy of National ID card, passport orany other form of identification
4. Referral letter from Accredited Medical Institution/Doctor
5. Quotation/invoice with total cost for medical procedure

N.B: Only non-life threatening medical procedures.
Our intervention will only cater for medical procedures the public health sector is unable to perform.
